|Fill out our form online, print it and include it with your material. Handwritten orders will be rejected. If you are accepting a written estimate, please print it out and send it along with your job.|
|Our forms are in PDF format to ensure uniformity and ease of use no matter what browser you have. All fields are interactive, with the exception of the signature and date lines that are to be filled out after printing.|

|Please include your e-mail address! We e-mail you when we ship your package. Any discrepancies in your order are handled through e-mail. We do not use the phone to clarify orders and confirm shipping information.|
|Write your name on each can, videotape or box that you submit. If your rolls are to be prepped and transferred, designate the order of compilation into lab rolls.|

||Unless instructed otherwise, our default Bono label will display, at a minimum, the customer name and runtime on videotapes and DVD-Rs. If your project is for a gift or festival submission please give explicit direction on the content of your label. Archives & collections should provide a sample of their label template.|
|Include complete credit card information, including expiration date and security code. Don't forget to sign the form once you have printed it out. A $5.00 credit card decline fee will be assessed if your credit card is declined. If for any reason you find it necessary to cancel your order after your credit card has been charged, a $25 administrative fee will be assessed.|
||In addition to the shipping company fee (USPS), all orders will be charged a $7.00 handling/packing fee or $10 per hard drive shipment.|
|In response to client
concerns to bring shipping costs down we have chosen the US Postal Service
as our default shipper. The rates are fairly uniform across the country and
are generally assessed by the weight. The average delivery time is running 2
- 3 days. As mentioned, a small handling fee is charged for all shipments.
We accept incoming packages from all major shippers.

client accounts. We will accept client FedEx, UPS
or DHL account numbers. A $2.95 fee is charged for clients providing a
client shipper number.
All UPS packages are sent without a signature being required. If you live in a high crime district and need the signature for delivery designation you must e-mail us with those specific directions. There is a $2.00 additional charge for this service.
We recommend that you ship your packages US Priority Mail with Delivery Confirmation so you will know that your package has arrived here safely.
